COVID-19 Self -Declaration

I / we hereby consent to attend the performance at the SMVRCH venue during the COVID -19 outbreak.

1. By my/our attendance at the performance, I/we declare/acknowledge/consent to the following:
   1. I/we have not within twenty one (21) days immediately preceding the Date of this Self Declaration:
      1. Experienced any symptoms commonly associated with COVID-19;
      2. Tested positive with the Coronavirus or been suffering from any symptoms of COVID-19 or have I/we been identified as a potential carrier of COVID-19;
      3. Been in any location/resided in/ traveled to any containment zone or hotspots as designated by the government/health authorities and/or any other place potentially infected with the COVID-19 as recognized by the government or health authority;
      4. Been in direct contact with or in the immediate vicinity of any person who I/we knew and/or now know to be infected with the COVID-19and/or as a potential carrier of COVID-19 or has been identified to be suffering from symptoms of COVID-19.
   2. I/we am/are aware of the risk to my/our physical well-being by being present at the SMVRCH venue by exposure to COVID-19 or other such illness.
   3. I/we have read and understood the safety measures being taken by SMVRCH and I/we am/are aware that in spite of the safety measures and adherence to guidelines issued by the government there could be a COVID-19 outbreak at the SMVRCH venue and/or I/we could contract COVID-19 and I/we shall not hold SMVRCH responsible for the same.
   4. I/we acknowledge that the organiser / producer / event manager / artist, reserves the right to cancel the performance at its sole discretion in the event of or suspicion of a COVID-19 outbreak/risk. Refund of the ticket amount is left to the discretion of the event manager/organiser of the event and SMVRCH and www.5oh9.com cannot be held responsible for the same.
   5. I/we consent to having my/our temperature taken by any representative of SMVRCH, provide any follow up information, wear a mask at all times while at SMVRCH venue and will adhere to the protocols/ norms specified by SMVRCH from time to time to prevent the spread of Coronavirus.
   6. I/we acknowledge and accept that this Self Declaration shall be deemed to be my/our consent for SMVRCH to store, record, and report this Self Declaration & Liability Release Waiver to the government or health authorities as may be necessary to prevent the further spread of the Coronavirus.
   7. With full knowledge of the risks involved, I/we hereby release, waive, discharge SMVRCH, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, including but not limited to cancelation of the performance that may be sustained by me related to COVID-19 while attending the performance while in, on, or around the SMVRCH venue or while using the facilities that may lead to unintentional exposure or harm due to COVID-19.
   8. By signing below I acknowledge that I/we have read the foregoing Self- Declaration and understand its contents; that I/we am/are at least eighteen (18) years old and fully competent to give my/our consent; That I/we have been sufficiently informed of the risks involved ; that I/we give my/our voluntary consent in signing this Self-Declaration as my/our own free act and deed with full intention to be bound by the same, and free from any inducement or representation.

This waiver will remain effective until laws and mandates relevant to COVID-19 are lifted.

I/we state that all information provided by me/us in this Self Declaration is true and correct to the best of my/our knowledge and belief.
